Job summary

VitalCaring Group in Oklahoma is seeking a PRN Weekend Home Health RN to lead care in patients' homes, ensuring starts of care and smooth weekend transitions. You will work autonomously, assess, plan, and adjust care in real time to achieve positive outcomes.

The role emphasizes high-quality, patient-centered nursing with strong teamwork and on-call responsibilities, EMR documentation, and driving hospital readmission reductions while delivering compassionate, top-of-license care.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate
