Job summary

VitalCaring is seeking a PRN Weekend Home Health Registered Nurse to lead Starts of Care and ensure smooth transitions home over the weekend. You will work autonomously, manage plans of care, and collaborate with a multidisciplinary team to deliver high-quality care in patients' homes.

The role requires an active RN license, CPR, and 1 year of RN experience (home health preferred). Flexible weekend scheduling offers meaningful patient impact and professional growth.

Qualifications

• Active RN license (or compact eligibility) + CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• BSN preferred in some postings; relevant certifications may be evaluated.

Responsibilities

• Conduct in-home assessments to evaluate clinical status and goals.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medications, treatments, and interventions.
• Coordinate across therapists, aides, and physicians as the case manager.
• Monitor patient progress and adjust care as conditions change.
• Educate patients and families on disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain accurate EMR documentation.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
