Job summary

VitalCaring Group in Oklahoma is seeking a PRN Weekend Home Health Registered Nurse to lead Starts of Care and ensure smooth transitions home over the weekend. You will work autonomously in patients' homes, delivering advanced nursing care and coordinating with multidisciplinary teams.

The role emphasizes top-of-licence practice, strong clinical judgment, and building meaningful relationships with patients and families.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum one year RN experience; Home Health/acute care preferred.
• Strong clinical judgment and independent decision making.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate status and goals.
• Develop and manage individualized care plans with physicians and teams.
• Deliver skilled nursing care including medications and treatments.
• Serve as case manager coordinating across therapists and aides.
• Monitor progress and adjust care based on clinical changes.
• Educate patients and families on disease management.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
