Job summary

VitalCaring Group in Kansas is seeking a Weekend Home Health RN to lead Starts of Care and manage patient transitions in the home setting. This PRN role offers clinical autonomy and the chance to impact patient outcomes from day one.

As the clinical lead, you will perform in-home assessments, develop care plans with physicians, deliver skilled nursing care, coordinate with therapists and aides, and document everything in the EMR while maintaining a strong caregiver–family relationship.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
