Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead Start of Care and weekend transitions home, delivering high-quality care in patients’ homes. You’ll exercise clinical autonomy within a supportive team while maintaining excellent patient relationships and documentation.

The role emphasizes strong clinical judgment, independent decision-making, and a patient-centered approach, with flexible weekend hours and a focus on outcomes and safety.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently; excellent communication skills.
• Proficiency with EMR systems and a valid driver’s license.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
