VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead care for patients in their homes and manage Starts of Care and weekend transitions. You’ll work autonomously, assess, decide, and adjust in real time, with strong support from a collaborative clinical team.

You’ll bring 1+ year RN experience (home health preferred), active RN license, CPR, and a commitment to patient-centered outcomes.

Qualifications

Active RN license and CPR certification required.
Graduate of an accredited nursing program.
Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
Strong clinical judgment and ability to think independently.
Excellent communication skills and patient-centered approach.
Proficiency with EMR systems and valid driver's license.

Responsabilités

Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
Develop and manage individualized care plans in collaboration with physicians and care teams.
Deliver skilled nursing care, including medication management, treatments, and interventions.
Serve as the case manager, coordinating care across therapists, aides, and physicians.
Monitor patient progress and make clinical decisions based on changing conditions.
Educate patients and families to support disease management and independence.
Coordinate care transitions and discharge planning to reduce readmissions.
Maintain timely, accurate documentation in the EMR.
Supervise and support Home Health Aides as needed.
Participate in on-call rotation as appropriate.
