Job summary

VitalCaring Group is seeking a Weekend Home Health Registered Nurse to serve as the clinical lead for your patients, delivering Starts of Care and guiding transitions home over the weekend.

You’ll perform in-home assessments, develop care plans, administer skilled nursing interventions, and coordinate with physicians and therapists to ensure safe, high-quality outcomes while maintaining autonomy and compassionate patient relationships and documenting care progress diligently in the EMR to

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Minimum one year RN experience; Home Health preferred.
• Graduate of an accredited nursing program.
• Strong clinical judgment and independent decision-making.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and interventions.
• Serve as case manager coordinating across therapists, aides, and physicians.
• Monitor patient progress and adjust plans based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
