Job summary

Atrinity Home Health, a part of LHC Group within the Optum family, is hiring a Physical Therapist to perform assessments, develop treatment plans, and deliver interventions in home health settings.

You will evaluate functional status, coordinate with physicians and nurses, document progress, educate patients and families, and support performance improvement within state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle access or reliable transportation.
• Ability to function in any home situation regardless of patient background.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports the patient's response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs family/caregivers regarding the plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
