Job summary

Atrinity Home Health is seeking a Physical Therapist to join our home health team. You will assess patient mobility, endurance, and pain, develop a treatment plan, implement interventions, and evaluate progress to enhance well-being.

Responsibilities include documenting progress, creating PT plans within state guidelines, and communicating with patients, families, and the care team to ensure goals are met.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to dependable vehicle or public transportation.
• Ability to function in home settings regardless of client background.

Responsibilities

• Evaluates patient functional status and needs; assists physician in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses and changes to supervising clinician.
• Treats patients to relieve pain and restore function to maximum levels.
• Instructs patient and family on plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes PT plan of care within state guidelines and documents outcomes.
• Reports outcomes, goals, and projected frequency of care.
