Job summary

UnitedHealth Group is seeking a Physical Therapist for home health to assess and evaluate patient care needs related to mobility and functional status. You will develop treatment plans, perform interventions, and monitor progress to improve patient well-being.

Responsibilities include evaluating function, delivering therapy in coordination with the care team, documenting progress, and educating patients and families. Sign-on bonus available for full-time PTs.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to dependable transportation.
• Preferred CPR certification.

Responsibilities

• Evaluates the patient's functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and in coordination with health care team.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care and progress towards goals.
• Visits patients as per Plan of Treatment; completes progress notes per policy.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
