Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification (preferred)

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and plan of care.
• Reports patient responses and changes to supervising nurse/physician.
• Treats to relieve pain, improve function, and maximize functioning.
• Instructs patient and family on plan of care and progress.
