Job summary

HealthPro Heritage in Wallingford, CT is seeking a Weekend Home Health Physical Therapist to join our team serving adults in home health settings with flexible scheduling options including Full Time, Part Time, and PRN.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.
• Licensure: Valid state licensure as a Physical Therapist, or license eligible.
• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.

Responsibilities

• Evaluate patients' physical conditions, mobility, strength, and flexibility.
• Provide therapeutic exercises, manual therapy, and educate patients and families on home routines.
• Maintain accurate records of evaluations, progress, and treatment plans.
• Collaborate with healthcare providers and participate in multidisciplinary meetings.
