Qualifications

• Ability to work independently and communicate effectively.

Responsibilities

• Provides service within the scope of practice per state laws and plan of care.
• Evaluates functional status, CMS-required aspects, and therapy needs; consults with physician.
• Observes, records, and reports patient response to treatment and condition changes.
• Conducts patient assessments using functional ability tests.
• Function in diverse home environments, respecting all clients.
