Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a vehicle or public transportation.
• Ability to work in home health environments regardless of client background.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists in development of the therapy Plan of Care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient progress to supervising staff.
• Treats patients to relieve pain and restore function.
• Informs patient and family about the plan of care and progress.
• Completes evaluations and documents progress per policy.
• Reports outcomes and anticipated frequency of care.

Skills
