Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred within 90 days of hire).

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists the physician in the development of the plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients, families and care team regarding plan of care, functional limitations and progress.
• Visits patients according to the Plan of Treatment and submits progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines; reports outcomes and frequency of care.
• Adapts care for home environments and diverse patient needs.
