Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health. You will assess patient needs, develop and implement treatment plans, and monitor progress to improve mobility and function in a home health setting.

Requires state PT licensure, valid driver’s license, and reliable transportation. CPR certification is preferred and benefits include a comprehensive package for eligible staff.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred) or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient functional status and PT needs within the plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervisor/physician.
• Treats patients to relieve pain, restore function, and maintain max functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients and documents progress notes per policy.
• Completes evaluation and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and frequency of care.
