Weekend Physical Therapist

BAYADA Home Health Care is seeking an experienced Weekend Physical Therapist to provide care to adult and geriatric patients in their homes and surrounding towns including Dedham, Canton, and Foxboro.

Prior home health experience is preferred but not required. As a home health physical therapist, you will be part of a multidisciplinary team providing skilled nursing and rehabilitative care to help clients maintain their independence.

Responsibilities include:

1. Making visits to clients in their homes
2. Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments
3. Developing and implementing individualized care plans with manual therapy, gait training, and other interventions
4. Assessing and revising care plans, collaborating with nursing staff
5. Educating clients and family members on rehabilitative activities to promote health and independence
6. Documenting observations and interventions accurately using a tablet

Qualifications include:

• Current Massachusetts Physical Therapist license
