The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and evaluate progress.



Responsibilities
  • Evaluate patients' functional status and physical therapy needs; consult with physicians on therapy plans.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Observe and report patient responses and condition changes to supervisors and physicians.
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based practices.
  • Educate healthcare team members, patients, and families about the care plan, limitations, and progress.
  • Visit patients within the service area and nearby agencies as needed.
  • Complete progress notes after each visit and submit them per policy.
  • Perform evaluations and develop the PT plan of care within state guidelines; report outcomes and care plans to supervisors within 24 hours.
  • Participate in staff meetings and committees as requested.
  • Supervise home health aides and PT assistants as allowed by policy and regulation.
  • Participate in weekend rotations as needed.
  • Prepare discharge summaries and periodic patient reports.
  • Provide in-service education as required.
  • Adhere to therapy standards and maintain licensure through ongoing education.
  • Perform other duties as assigned.


Education and Experience

Licensure Requirements


  • Current Physical Therapy license in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
