Job description

Nurse On Call is hiring for a full-time Home Health Physical Therapist to join our passionate team in Lincoln, RI!

Territory includes Providence, Pawtucket, and East Providence.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities
  1. Evaluate the patient's functional status, all relevant body systems for CMS documentation, and physical therapy needs. Consult with the physician for developing the therapy plan of care.
  2. Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  3. Observe and report patient responses to treatment and any condition changes to the nurse supervisor and/or physician.
  4. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based practices.
  5. Instruct and inform healthcare team members, patients, and families/caregivers regarding the plan of care, limitations, and progress.
  6. Visit patients within their service area and nearby agencies as needed.
  7. Complete progress notes for each visit and submit them per policy.
  8. Complete patient evaluations and develop the PT plan of care within state guidelines. Report outcomes and projected care frequency within 24 hours of evaluation.
  9. Participate in staff conferences and committees as requested.
  10. Supervise home health aides and PTAs as per policy and state regulation.
  11. Participate in weekend rotations as needed.
  12. Prepare discharge summaries or reports on patient's condition at least every 60 days.
  13. Provide in-service education as required.
  14. Adhere to LHC Therapy Standards of Practice.
  15. Maintain current clinical knowledge and licensure through ongoing education.
  16. Perform other duties as assigned.
Education and Experience
Licensure Requirements
  • Current Physical Therapy license in RI
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
