The Physical Therapist in Home Health assesses and evaluates patient needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and evaluate progress.



Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and consult with physicians for care planning.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Monitor and report patient responses and condition changes to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, restore or maintain function using evidence-based techniques.
  • Educate patients, families, and healthcare team members about the care plan, limitations, and progress.
  • Visit patients across the service area and nearby agencies as needed.
  • Complete progress notes after each visit and submit them per policy.
  • Complete patient evaluations and develop the PT plan of care within state guidelines, reporting outcomes and projected care frequency within 24 hours.
  • Participate in staff meetings and committees as requested.
  • Supervise home health aides and PTAs as permitted by policy and regulation.
  • Participate in weekend rotations as needed.
  • Prepare discharge summaries and periodic patient condition reports.
  • Provide in-service education upon request.
  • Adhere to LHC Therapy Standards of Practice.
  • Maintain and enhance clinical knowledge and licensure through ongoing education.
  • Perform other duties as assigned.


Education and Experience

Licensure Requirements


  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
