Weekend Home Health Physical Therapist

Per Diem or Part-Time

Flexible Weekend Schedules Available

BAYADA Home Health Care is currently seeking an experienced Weekend Physical Therapist for a Per Diem or Part-Time opportunity performing home health visits for our office located in Providence, Rhode Island. Flexible weekend schedules are available. You will provide rehabilitative physical therapy to adult and geriatric patients in their homes throughout Rhode Island.

One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary team as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

Qualifications

• Current Rhode Island Physical Therapist (PT) license.
• Minimum of one year recent work experience as a Physical Therapist.
