The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) on a PRN basis to join weekend staffing with flexible options for other shifts. The ideal candidate holds an accredited PT degree with an active license and will supervise PTAs, perform patient assessments, develop individualized treatment plans, and document progress while collaborating with the care team to ensure high-quality geriatric rehabilitation.

Experience in long-term care is preferred.

Formación

• Graduated from an accredited Physical Therapy program.
• Must hold a valid professional license in state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation writing skills.
• Conduct evaluations, progress notes, recertifications and discharge documents.
• Collaborate with all disciplines for patient-centered care.
• Deliver exceptional patient service with clinical excellence.
• Report on-time, adhere to scheduled hours, maintain professionalism.
• Perform other duties as assigned by Director of Rehabilitation.
• Comply with confidentiality, HIPAA and residents' rights.

Responsabilidades

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with specific goals.
• Set realistic and achievable goals for patients and document status.
• Educate patients and families in an appropriate physical therapy method.
