The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) to join our facility on a PRN basis, with emphasis on weekend staffing. Candidates with flexible availability will be considered.

The PT will assess referred patients, design treatment plans, supervise Physical Therapy Assistants, document progress, and educate patients and families in rehabilitation methods. HIPAA compliance and patient-centered care are essential.

Qualifikationen

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.

• Assess referred patients requiring rehabilitation.
• Establish and administer treatment programs with goals.
• Document and record patient condition and progress.
• Educate patients and families in physical therapy methods.
• Collaborate with all disciplines to provide patient-centered care.
