Job summary

Providence at Home with Compassus is seeking a Clinical Care Partner I to coordinate safe, efficient transitions of care from hospital to home-based services. This in-person role involves bedside engagement, interdisciplinary collaboration, and active participation in discharge planning.

You will work with physicians, case management, and families to ensure high-quality, timely care and appropriate post-acute pathways. Required: Associate’s degree; LPN/LVN license and CPR.

Qualifications

• Associate’s degree required; Bachelor’s degree preferred.
• Active LPN/LVN or equivalent designation for state of employment.
• Current CPR certification.
• Experience with EMR systems (e.g., Epic) and referral platforms.

Responsibilities

• Coordinate safe, patient-centered transitions of care from hospital to home-based services.
• Evaluate post-acute care suitability using clinical and environmental factors.
• Collaborate with physicians and care teams for level-of-care decisions.
• Arrange home health, hospice, DME, medications, and follow-up care.
• Educate patients and families on post-acute options and expectations.
• Maintain strong relationships with hospital teams and post-acute providers.
