Job summary

Providence at Home with Compassus is seeking a Clinical Care Partner III to coordinate safe transitions from hospital to home-based care. You will evaluate patients for appropriate post-acute services and support discharge planning with physicians, case management, families, and post-acute teams.

You will educate patients on care options, collaborate with multiple disciplines, and ensure accurate documentation in the EMR while driving value-based care outcomes.

Qualifications

• An accredited degree in nursing (RN) or Master of Social Work (LCSW) or Master’s in Speech Language Pathology.
• Experience in care coordination or discharge planning is preferred.
• Active licensure or eligibility in the state of employment.

Responsibilities

• Evaluate patients for home-based and post-acute care services.
• Coordinate timely, safe hospital discharge planning and transitions.
• Educate patients and families on post-acute care options and expectations.
• Manage referrals between hospital teams and post-acute providers.
