Ascension at Home together with Compassus is seeking a Clinical Care Partner III to coordinate safe, efficient post-acute transitions for hospitalized patients. This in-person role focuses on discharge planning, home-based care referrals, and interdisciplinary collaboration with physicians, case management, and families.

You will evaluate patients for post-acute services, arrange home health or hospice as needed, and support value-based care goals while reducing length of stay and readmissions.

Formación

• RN or LMSW or LCSW required.
• Preferred 2–3 years in care coordination, discharge planning, or healthcare services.
• Experience with EMR systems (Epic) and referral platforms.

Responsabilidades

• Coordinate safe transitions of care and discharge planning.
• Assess post-acute home-based care needs and coordinate services.
• Collaborate with physicians, case management, and families.
• Ensure timely handoffs to post-acute providers.
