Qualifications

• RN, LCSW, or Speech Therapist license required.
• 2–3 years preferred in care coordination, discharge planning, or post-acute care.

Responsibilities

• Coordinate safe hospital-to-home transitions and post-acute care planning.
• Educate patients and families on post-acute options and expectations.
• Manage referrals and ensure timely handoffs to post-acute providers.
• Collaborate with physicians, case management, nursing teams and discharge planners.
• Support hospice/GIP coordination when applicable and document activities in EMR.
