Qualifications

• Ability to read, write and communicate effectively in daily tasks.
• Physical stamina to assist with lifting and mobility tasks.

Responsibilities

• Assist clients with basic living skills and daily tasks.
• Provide transportation to activities and appointments.
• Document accurately in logs and required forms.
• Follow Plan of Care and monitor medications per guidelines.
• Maintain confidentiality and professionalism; HIPAA compliant.
