Job summary

Vernon Rehabilitation and Healthcare Center in Vernon, CT is seeking a Charge Nurse for weekends only to oversee a weekend Baylor unit, providing patient care, supervising staff, and coordinating with the interdisciplinary team.

Qualifications

• Active LPN or RN license in CT.
• High School diploma or equivalent.
• Experience in long-term care or rehabilitation preferred.

Responsibilities

• Supervise and coordinate care on the weekend Baylor unit.
• Collaborate with interdisciplinary team to develop care plans.
• Provide direct patient care and assist with medications and treatments.
• Monitor resident conditions and report changes to clinicians.
• Maintain accurate records of care and treatment plans.
• Participate in quality improvement and policy development.
• Demonstrate strong communication with residents, families, and staff.
