Compassus is seeking a Remote Home Health Registered Nurse - Triage to triage after-hours calls and support referrals for home health and hospice patients. The role involves coordinating with physicians, scheduling admissions and visits, and supporting field staff on weekends (Saturday and Sunday, 9:00 AM–7:00 PM Central Time).

Required are an Associate or Bachelor’s degree in Nursing, an active RN license in employment states, and CPR certification. Prior home health experience is preferred.

Formación

• Associate or Bachelor’s degree in Nursing is required.
• One year RN experience in clinical care or home health preferred.
• Active and unencumbered RN license in state(s) of employment; CPR certification required.

Responsabilidades

• Accepts and facilitates new referrals for home health and hospice patients.
• Liaises with referral sources, patients, and families.
• Provides accurate and responsive service to calls during weekend hours.
• Collaborates with physicians and obtains orders as needed.
• Schedules patient admissions and visits with field staff; monitors telehealth as needed.
