Qualifications

SNF, rehab, or healthcare admission experience preferred.
Detail-oriented, organized, and capable of handling multiple parts.
Empathetic, clear communication with professionalism.
Proficient in Microsoft Office and relevant healthcare portals.

Responsibilities

Be the primary contact for referrals, calls, walk-ins, and inquiries.
Coordinate admission docs, insurance verification, and pre-admission authorization.
Communicate with hospitals, case managers, residents, families, and staff.
Enter data accurately and update the PCC CRM promptly.
Support tours, follow-up on referrals, and conversion activities.
Maintain excellent customer service at every touchpoint.
Collaborate with nursing, therapy, and admin teams for smooth admissions.
Ensure rooms are ready for admitting residents daily.
Assist with marketing events and community outreach.
