Golden Heights Healthcare in Daly City, CA is seeking a Weekend Admissions Coordinator to greet referrals, residents, families, hospitals, and partners, and to manage admission workflow.

You will coordinate documentation, verify insurance, and use PCC and hospital portals to ensure accurate data and timely communication. The role emphasizes customer service, detail orientation, and collaboration with nursing and administrative teams on Saturdays and Sundays.

Qualifications

• Experience in SNF, rehab, or healthcare admissions is strongly preferred.
• Detail-driven: organized, accurate, and able to manage multiple moving parts.
• Customer-focused: communicate with empathy, clarity, and professionalism.
• Tech-savvy: proficient with Microsoft Office, PCC, insurance portals and hospital referral portals.

Responsibilities

• Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries.
• Coordinate admission documentation, insurance verification, and pre-admission authorization.
• Communicate clearly with hospitals, case managers, residents, families, and internal departments.
• Ensure accurate data entry and timely updates in Point Click Care CRM.
• Facilitate facility tours, referral follow-up, and conversion strategies.
• Maintain strong customer service standards at every touchpoint.
• Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions.
• Ensure empty rooms are admit and tour ready daily.
• Assist with marketing events, community outreach, and relationship development.

$28.00–$30.00/hour | Saturday & Sunday | 9:00 AM–4:30 PM

Golden Heights Healthcare in Daly City, California is seeking a Weekend Admissions Coordinator to join our team. In this role, you’ll be a key point of contact for referrals, residents, families, hospitals, and healthcare partners. Your organization, communication, and professionalism will help ensure a smooth and welcoming admission experience.

Key Responsibilities:

• Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries.

• Coordinate admission documentation completion, insurance verification, and pre-admission authorization

• Communicate clearly and professionally with hospitals, case managers, residents, families, and internal departments.

• Ensure accurate data entry and timely updates in Point Click Care CRM

• Facilitate facility tours, referral follow-up, and participate conversion strategies.

• Maintain strong customer service standards at every touchpoint.

• Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions.

Education/Requirements

You’ll be a great fit if you are:

• Collaborative – A team player who works well with nursing, clinical, and leadership staff.
