Evergreen Healthcare Group is seeking a Weekend Admissions Coordinator to help welcome new residents and support families through the admission process. The role involves managing referrals, coordinating the admissions workflow, and serving as a bridge between the facility, hospitals, and community every weekend.

The position requires strong communication, organizational skills, and familiarity with Medicare/Medicaid processes.

Qualifications

• Previous experience in admissions, healthcare, or SNF preferred.
• Knowledge of Medicare, Medicaid, and managed care processes.
• Strong communication and interpersonal skills.
• Excellent organizational and multitasking abilities.
• Proficient in basic computer systems and EMR platforms.
• Ability to work in a fast-paced environment.

Responsibilities

• Manage incoming referrals and coordinate the admissions process from start to finish.
• Communicate with hospitals, case managers, physicians, and families.
• Verify insurance coverage and obtain necessary authorizations.
• Conduct facility tours for prospective residents and families.
• Maintain accurate admission records and documentation.
• Collaborate with clinical and administrative teams to ensure smooth transitions.
• Support census development and marketing initiatives.
