Empath Health is seeking a director-level leader to oversee the Access Team within the Admission–Care Navigation Department in Florida. The role requires RN licensure and hospice-management experience, with a strong emphasis on coordinating patient access and care navigation while maintaining regulatory standards.

Travel between multiple locations is required, and the successful candidate will lead coaching, training, and performance feedback to staff, ensuring the organization delivers

Qualificações

• Education and/or Experience: A.S. degree required - Bachelor’s degree (B.A.) from four-year college or university and Master’s degree preferred.
• Two years Hospice experience with at least two years of management experience preferred.
• At least five years total years experience as an RN preferred.
• Licensure: RN.
• Must have reliable transportation and travel between multiple locations.

What You’ll Need

Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file. This position requires travel between multiple locations.
