Job summary

Tidewell Hospice, part of Empath Health, seeks an experienced RN to provide after-hours care to patients in home, facility, or hospital settings on weekends and evenings. You will work under the Triage team and physician supervision to relieve symptoms and support families.

The role requires a Florida RN license, CPR, and at least 1 year of acute care experience; CHPN is encouraged. BSN/MSN preferred; hospice experience strongly preferred.

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid CPR certification required.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; hospice RN preferred.
• CHPN certification encouraged within three years of employment.
• BSN or MSN preferred.
• An equivalent combination of professional experience and education may be considered.

Responsibilities

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team.
• Under physician direction, performs patient assessment and manages discomfort with palliative care techniques.
• Uses nursing interventions to maximize patient comfort and quality of life for patient/family unit.
• Attends patient deaths and provides bereavement support as indicated.
• Documents patient services in EMR reflecting care plan and outcomes.
• Reviews and updates Plan of Care and orders to meet patient/family needs.

What You’ll Do

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team (in in the patient home, long term care facility, assisted living facility, hospice house or hospital)
• Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
• Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care.
• Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
• Reviews and updates the Plan of Care, physician orders, MAR/Pharmacy profile and other records to ensure patient/family needs are met.
