Tidewell Hospice, part of Empath Health, is hiring a Continuous Care LPN for 8:00 PM–8:00 AM shifts with at least two weekend nights per month. The role covers Manatee, Charlotte, DeSoto, and Sarasota counties, with mileage reimbursement.

You will work under an RN case manager, delivering bedside care and reporting changes in patient condition. Requirements include graduation from an approved nursing program and a Florida LPN license, plus ideally one year of acute care or hospice experience.

1:1 Patient Ratio | (8:00 PM – 8:00 AM), Must include 2 weekend nights per month

What You’ll Do

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.
