Job summary

People Incorporated Mental Health Services seeks Case Manager IIs for Waiver Case Management in Hennepin County. You will develop person-centered Support Plans and authorize services to help clients live independently and in the community.

Roles involve coordinating services, maintaining documentation, and building long-term relationships with clients while operating with hybrid scheduling and a manageable caseload.

Qualifications

• Bachelor’s degree in a relevant field and 2,000 hours of supervised experience with adults with mental illness.
• Valid driver’s license with a clean driving record and current auto insurance.
• On-site presence in the Twin Cities, Minnesota, with hybrid work option.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain a collaborative relationship with clients through positive, proactive communications.
• Assess client needs and ensure continuity of care.
• Maintain regular contact with collaborative partners.
• Make referrals and authorize services based on client needs and outcomes.
• Maintain records and document all provided services.
• Document all interventions and information to the Support Plan per standards.

Case Manager II - Waiver Case Management (Hennepin County)People Incorporated is seeking Case Manager II's to join our Waiver Case Management- Hennepin County team to work with clients who are diagnosed with chronic mental illness or other ongoing health conditions. We connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management. Our Case Managers see clients twice annually and develop long term relationships by developing dynamic service plans. Our staff operate autonomously, creating their own hybrid schedules after completing initial training.
