Qualifications

• Bachelor’s Degree in behavioral science or related field.
• Master’s Degree in behavioral science or related field.
• 2,000 hours of supervised experience working with adults with severe and persistent mental illness.

Responsibilities

• Perform functional assessments that lead to individual plan development and access to needed services.
• Regularly review plan effectiveness and adjust as needed to maximize support.
• Assess client needs and ensure continuity of care.
• Make referrals to resources based on assessment and client needs.
• Maintain daily records for continuity of service and funding reports.
• Document all interventions and information to treatment plans.
• Attend client monthly meetings to monitor services.

Education

Bachelor’s Degree in behavioral science or related field
