Job summary

Quality Home Health in Tennessee seeks an experienced Registered Nurse to supervise home health visits and lead RN, LPN and HHA teams. The role requires coordinating patient schedules, directing direct care per the care plan and ensuring compliant documentation.

The nurse will communicate with physicians, families, and the RN Team Leader, participate in conferences, and oversee care coordination, calendar management, and regulatory adherence.

Qualifications

• Licensed Registered Nurse in Tennessee.
• Ability to provide direct patient care and on-call duties.
• Experience or knowledge of home health care programs and visits preferred.
• Computer data entry and word processing skills desirable.

Responsibilities

• Accepts patient care reports and assignments from the RN Team Leader and ensures timely documentation.
• Obtains weekly visiting schedule for all assigned patients and conducts visits per schedule.
• Notifies RN Team Leader of schedule changes and provides advance notice as possible.
• Maintains an effective visit route and provides direct patient care per plan of care.
• Communicates with RN Team Leader, physicians and family to coordinate care and follow-up.
• Assists in maintaining calendar, disaster codes, and lab due dates.
• Supervises Home Health Aide and LPN per plan of care and regulatory requirements.
• Completes visit reports, OASIS assessments, care plan revisions, and progress notes.
• Attends team conferences and provides progress reports to physician every 60 days.
