Job summary

Quality Hospice Care is seeking a dedicated Registered Nurse to provide hands-on care for home health patients in Tennessee. You will supervise RNs, LPNs, and Home Health Aides, ensuring adherence to the plan of care and Medicare/state regulations.

Qualifications

• Must hold Tennessee RN license and be able to provide direct patient care.
• Experience with home health visits preferred.
• Basic data entry and word processing skills desirable.

Responsibilities

• Accept patient care reports and assignments from RN Team Leader and coordinate visits.
• Obtain weekly visiting schedule and conduct visits as scheduled; notify RN Leader of changes.
• Maintain an efficient visit route and provide direct patient care per plan of care.
• Communicate with RN Leader, physicians, and family to coordinate care and follow up.
• Assist in maintaining calendar, disaster codes, and lab/test follow ups as needed.
• Supervise Home Health Aide and Nurse roles according to policy and care plans.
• Make patient visits per physician orders and document visit reports, care plans, and revisions.
• Be available for PRN and on-call visits and participate in team conferences.
• Attend meetings, maintain confidentiality, and participate in overtime or special projects.
