Job summary

Quality Private Duty Care is seeking a licensed practical nurse to deliver hands-on home health care for assigned patients in a private duty setting. The RN Team Leader will coordinate visits and ensure documentation is completed accurately.

The ideal candidate is licensed in Tennessee, can provide direct patient care, and is available for on-call; prior home health experience and basic computer skills are preferred to maintain patient records and calendars.

Qualifications

• Licensed Practical Nurse license in Tennessee.
• Ability to provide direct patient care and on-call.
• Experience in home health visits preferred.
• Computer data entry and word processing skills desirable.

Responsibilities

• Accepts patient care reports and coordinates with the RN Team Leader to complete visits and documentation.
• Obtains weekly visiting schedule and conducts visits per schedule.
• Notifies RN Team Leader of schedule changes with advance notice.
• Establishes efficient visit routes and provides direct patient care per the plan of care.
• Communicates with RN Team Leader, physicians, and family to coordinate care and follow-up.
