Job summary

Qualifications

• Registered nurse licensed in Tennessee.
• Ability to provide direct patient care and accept on-call duties.
• Experience with home health visits and data entry/word processing.

Responsibilities

• Coordinate with RN Team Leader to complete visits and document accordingly.
• Obtain weekly visiting schedule and follow it, with daily contact for changes.
• Communicate with physicians and family to coordinate care.
• Supervise Home Health Aide and Nursing staff per plan of care.
• Complete visit reports, OASIS, and care plan revisions.
• Attend team conferences and provide progress reports every 60 days.
• Maintain calendar, disaster codes, and lab due dates.
