• Accepts patient assignments from the RN Team Leader.
• Coordinates visits and documents timely.
• Maintains schedule and notifies RN Leader of changes.
• Provides direct patient care per plan of care.
• Communicates with physicians, RN Leader, and family.
• Keeps accurate visit and care plan documentation.
• Rotates on-call to support patient needs.
• Maintains confidentiality and professional conduct.
