Job summary

Quality Private Duty Care is seeking an LPN Staff Visiting Nurse to deliver hands-on patient care in home health visits across assigned patients. You will execute the plan of care, document visits, and coordinate with the RN Team Leader.

You must be licensed in Tennessee, hold a valid driver's license, and be able to provide care and be on-call. The role emphasizes accurate documentation, timely communication with physicians and family, and adherence to organizational policies.

Qualifications

• Licensed Practical Nurse, currently licensed in Tennessee.
• Current valid driver’s license.
• Must be able to provide direct patient care and take on-call.
• Working knowledge of the home health care program and previous experience providing home health visits preferred.
• Computer data entry and word processing skills desirable.

Responsibilities

• Accepts patient care reports and assignments from the RN Team Leader.
• Obtains a weekly visiting schedule for all assigned patients and conducts visits per schedule.
• Notifies the RN Team Leader of any anticipated problems with the posted schedule.
• Establishes and maintains an effective visit route and provides direct patient care per plan of care.
• Provides frequent communication to the RN Team Leader, physicians, other team members, and the patient/family to coordinate changes in patient care.
• Maintains a complete and accurate calendar book including disaster codes and due dates for lab work.
• Makes assigned patient care visits per physician orders.
• Completes supporting documentation (visit reports, care plans, meds, etc.).
• Is available for prn and routine visits and rotates on-call assignments when requested.
• Attends team conferences and provides progress reports every 60 days.
• Oversees timely requisition of indicated patient supplies.
• Obtains verification of physician licensure.
• Adheres to organizational policies and seeks guidance when unclear.
• Maintains confidentiality of business information.
• Attends inservices or meetings as assigned.
• Participates in special projects and overtime when requested.
• Maintains professional appearance and timeliness.
