Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must be licensed in the state where practicing, or per nurse licensure compact rules.
• Must have at least two years of nursing experience.
• Must have one year experience in home health or hospice.
• Must understand federal, state, and local laws and regulatory guidelines governing home care operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate the overall delivery of home health services to patients per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate collaboration among physicians, referral sources, patients, caregivers, and employees.
