Job summary

Jobgether is seeking a Nurse Practitioner or Physician Assistant to join a patient-centered virtual care team in the United States. You will conduct video consultations, assess symptoms, and develop personalized treatment plans for acute and chronic conditions, including mental health support.

Role requires a valid APP license with FNP compact preferred, at least five years of independent practice, and experience with telehealth.

Qualifications

• Active, unrestricted APP license; FNP compact license preferred.
• At least five years of independent patient-care experience in family/primary/urgent care.
• Experience delivering care via telehealth with digital tools.
• Ability to coordinate treatment plans with physicians and teams.
• English fluency; bilingual English-Spanish preferred.

Responsibilities

• Conduct virtual consultations via video and secure chat to assess symptoms and medical histories.
• Develop personalized treatment plans and orders, including labs and prescriptions.
• Coordinate care plans and follow-up care with physicians and other clinicians.
• Provide ongoing virtual care for acute, chronic, and mental health needs.
• Collaborate with the care team to ensure continuity of care.
• Adapt to virtual care challenges with patient-centered solutions.
