Job summary

Jobgether seeks a Nurse Practitioner or Physician Assistant to join a patient-centered virtual care team in the United States. You will deliver comprehensive care to patients via video consultations and secure messaging, addressing acute illnesses, chronic conditions, and mental health concerns while focusing on accessibility and compassionate service.

You will diagnose, develop individualized treatment plans, order labs, coordinate care with physicians, and electronically prescribe medications

Qualifications

• Active, unrestricted license as an Advanced Practice Provider (APP).
• FNP compact license required for Nurse Practitioner candidates.
• Experience delivering patient-facing care via digital platforms (telehealth).
• Experience coordinating treatment plans with physicians and healthcare teams.

Responsibilities

• Assess patient needs via virtual consultations (video and chat).
• Provide clinical diagnoses using remote tools and patient history.
• Develop and coordinate care plans including labs and follow-ups.
• Prescribe medications electronically and explain usage and precautions.
• Deliver ongoing care for acute, chronic, and mental health conditions virtually.
• Collaborate with physicians and other providers to coordinate care.
• Identify solutions to virtual care challenges in a digital environment.
• Communicate clearly and empathetically with patients and colleagues.
