Local Infusion is seeking a licensed Nurse Practitioner to join our remote, patient-facing neurology and memory care team. This full-time role delivers telehealth care to Local Infusion patients, partnering with physicians to improve access and timely treatment.

You will provide direct clinical care, conduct assessments, develop care plans, and educate patients and families while maintaining EMR documentation in compliance with regulations.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

KEY ACCOUNTABILITIES

• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
