Local Infusion is seeking a Nurse Practitioner (Neurology and Memory Care) for a remote, patient-facing telehealth role licensed in New Hampshire. You will deliver direct care to Local Infusion patients, collaborate with physicians, and enhance access to treatment.

Responsibilities include assessments, care plan development, ongoing treatment management, and patient education for neurological and memory disorders. Strong documentation in the EMR is essential.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth

Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies

Partner with our neurologist(s) and broader care team to ensure continuity of care

Educate patients and families on diagnoses, treatment plans, and disease progression

Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred

2+ years of clinical experience in neurology, memory care, and/or dementia care settings

Strong clinical judgement and clinical documentation skills

Comfortable managing a full patient panel independently with physician collaboration

Excellent communication skills with patients facing cognitive decline and their caregivers
