Job summary

Local Infusion is seeking a licensed Nurse Practitioner to join our remote, patient-facing telehealth team focused on neurology and memory care. You will deliver direct care to patients, develop care plans with physicians, and educate patients and families about diagnoses and treatment options.

Ideal candidates hold an unrestricted NH NP license, geriatrics or family practice certification, and have 2+ years in neurology/memory care.

Qualifications

• Active NH NP license; geriatrics/adult-gerontology/family-practice certification preferred.
• 2+ years of clinical experience in neurology, memory care or dementia settings.
• Ability to conduct telehealth assessments and prescribe therapies.
• Strong documentation of clinical care in EMR per regulatory requirements.

Responsibilities

• Provide direct clinical care to patients via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment with supervising physicians including prescribing therapies.
• Partner with neurologist(s) and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatments, and disease progression.
• Maintain accurate clinical documentation in the EMR.

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

• Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
